Extract confirming your premarital surname
We obtain an extract from the Ukrainian civil registry that confirms your premarital (maiden) surname — anywhere in Ukraine, without you travelling.
When you need the extract
A diploma in your maiden name
A university or employer abroad sees one surname in your diploma and another in your passport — and asks for a document that links them.
The marriage certificate is lost
It is usually the marriage certificate that confirms the change of surname. If you no longer have it, the registry extract does the same job.
Bank, inheritance, pension
A foreign institution checks that the account holder, heir or applicant is the same person as in the old documents.
Surname changed back after divorce
After a divorce the surname changed again, and the full chain is needed.
What we need from you
- A scan or photo of your passport
- Your surname before and after marriage
- The approximate date and place where the marriage was registered
- A copy of the marriage certificate if you still have it
- The address the finished document should be sent to
We prepare everything required to approach the civil registry office on your behalf. Your presence in Ukraine is not needed.
How it works
- You send the details — we check that they are enough to find the registry entry.
- We prepare the documents needed for the request.
- We apply to the civil registry office where the marriage was registered.
- We collect the extract and, if you need it, have the apostille added straight away.
- The finished document is sent to your address in Ukraine or abroad.
Price and timeline
The premarital surname extract with an apostille — 7,200 UAH. Without an apostille — 6,000 UAH. Two to two and a half weeks in both cases. This is the final price for one document: the request, official fees, preparation and the apostille are all included. Only delivery is paid separately — from 810 UAH, depending on the country.
Extract, apostille and translation in one order
To be used abroad, the extract usually needs an apostille from the Ministry of Justice and a notarized translation. We do it all in one order — faster and cheaper than each step separately.
Tell us which country and which authority you are submitting to. Requirements differ: some accept a marriage certificate, others want this registry extract specifically.
Questions people ask most
How is the extract different from a marriage certificate?
A certificate is a document issued to you at some point. The extract is generated from the registry now, and it shows which surnames you had before the marriage. If the certificate is lost or incomplete, the extract solves the problem.
The marriage was registered in Soviet times. Can you find it?
Usually, yes. Old entries are searched in the archive — it takes longer, but it works.
How much does it cost?
7,200 UAH with an apostille, 6,000 UAH without — per document. The price is final; only delivery from 810 UAH is paid separately.
