What we translate most often

Form 086/o

The medical certificate for admission to an educational institution. Needed for study abroad and some exchange programmes.

Vaccination record 063/o

For a school or kindergarten abroad: vaccine names, dates and batch numbers. The table stays a table, so a doctor finds what they need quickly.

Discharge summaries and tests

Discharge summaries, doctors’ conclusions, examination results — for treatment abroad, an insurer or a visa.

How we handle medical texts

  • Latin and abbreviations are expanded and rendered in the standard terminology of the target language.
  • Doctors’ handwriting is read carefully. Anything that truly cannot be read is marked in the translation, not guessed.
  • We translate, we do not interpret. Diagnoses and conclusions are rendered exactly as in the original, without comments of our own.
  • Names as in your international passport, letter for letter. Send a photo of the passport with the certificate.
Medical certificates are not apostilled — neither form 086/o nor 063/o. Foreign institutions receive a translation certified by a notary. If you are told an apostille is required, write to us and we will advise on what can be done in your case.

How it works

1Send a photo of the certificate — we confirm the price and turnaround
2We translate — usually within one business day
3A Ukrainian notary certifies the translator’s signature
4Collect at our office or receive by Nova Poshta

A photo is enough for a quote. Notarization needs the original: bring it to our office or send it by Nova Poshta — we return it the same way.

Frequently asked questions

How much does a medical certificate translation cost?

Translation from 500 UAH plus 500 UAH for notarization. Multi-page discharge summaries and test results are priced by volume — we confirm the amount from a photo.

Does a 086/o or 063/o certificate need an apostille?

No, medical certificates are not apostilled. They are submitted with a notarized translation.

How long does it take?

Usually one business day. Urgent same-day translation is charged at double rate.

How do you translate vaccine names?

By the trade name as in the original, plus the vaccine type, so a foreign doctor can see what it protects against. Dates and batch numbers are carried over unchanged.

Can I order without coming to Zhytomyr?

Yes. Send photos by Telegram, WhatsApp or email, and the original by Nova Poshta. We send the finished translation back the same way, within Ukraine or abroad.

The exception is any service that needs the original document or your personal presence. We tell you this up front, before you pay. How we work remotely →
