Ukrainian name change certificate from abroad

We obtain a duplicate of your name or surname change certificate from any civil registry office in Ukraine. You do not need to travel.

In short. This certificate proves that the person with the old name and the person with the new name are the same. A duplicate with an apostille costs 7,200 UAH and takes 2–2.5 weeks; the price is final.
In practice it is faster. The duplicate itself is usually ready in 3–4 working days. We quote 2–2.5 weeks with a margin — including the apostille, air-raid alerts and the registry office’s response time.

When you need this document

Documents in different names

Your diploma is in the old surname, your passport in the new one. A foreign authority wants proof that this is one person and will not accept the documents without it.

The certificate is lost

The name change was registered long ago, and the certificate itself was lost, burnt or left behind in occupied territory.

A full chain for a foreign authority

A bank, university, immigration office or pension fund asks you to trace every name change from birth to today.

Old form or lamination

An apostille cannot be placed on a laminated certificate, and old forms with handwritten amendments are often refused.

What we need from you

  1. A scan or photo of your passport
  2. Your full name before and after the change
  3. The approximate date and place where the change was registered
  4. A photo of the old certificate if you still have one — it makes the search faster
  5. The address the finished document should be sent to

We prepare everything required to approach the civil registry office on your behalf. Your presence in Ukraine is not needed.

How it works

  1. You send the details — we check that they are enough to find the registry entry.
  2. We prepare the documents needed for the request.
  3. We apply to the civil registry office where the name change was registered.
  4. We collect the duplicate and, if you need it, have the apostille added straight away.
  5. The finished document is sent to your address in Ukraine or abroad.

Price and timeline

A name change certificate with an apostille — 7,200 UAH. Without an apostille — 6,000 UAH. Two to two and a half weeks in both cases. This is the final price for one document: the request, official fees, preparation and the apostille are all included. Only delivery is paid separately — from 810 UAH, depending on the country.

Not everyone needs an apostille. If the document is for use in Ukraine, or in a country that has a legal assistance treaty with Ukraine, it is not required — then it is 6,000 UAH. Tell us where you are submitting it and we will say which option is yours — check your country.
No consulate visit. All we need are copies of your documents and answers to a few questions — we prepare everything else. A Ukrainian consulate appointment often means a month of waiting, and it is not part of our timeline because it is not needed.

Duplicate, apostille and translation in one order

If the document is going abroad, it makes sense to do everything together: the duplicate, the apostille from the Ministry of Justice and a notarized translation. One order, one delivery, no shipping the document back and forth.

A marriage certificate is often requested together with this one — it is the document that explains a change of surname after marriage. Tell us where you are submitting your documents and we will say which set they expect.

Ordering from abroad? Message us on Telegram or WhatsApp, or send an email — from any country, at any time. We reply during Kyiv office hours with the exact price and timeline for your case. Pay by card or PayPal; the document reaches you by courier.

Questions people ask most

My surname changed when I got married. Do I need this certificate?

No. A change of surname after marriage is confirmed by the marriage certificate. A name change certificate is issued when the name or surname was changed through a separate procedure.

I changed my name twice. What should I do?

We obtain a document for each change — together they form the chain the foreign authority is asking for.

How much does it cost?

7,200 UAH with an apostille, 6,000 UAH without — per document. The price is final; only delivery from 810 UAH is paid separately.
